Skin needling or microneedling refers to a few different treatments. These include the dermaroller, microneedling pens such as the dermapen and more advanced skin treatments such as radiofrequency Microneedling (rf microneedling).  A range of skin concerns is treated with skin needling. These include face needling for acne scars and skin texture concerns, and microneedling for stretch marks, hair loss and even cellulite treatment. What can be achieved with face needling, and what specific skin concerns are treated? What are the primary treatment modalities used for skin needling, and what is the history of this fascinating skin therapy?

 

What can be achieved with face needling?

Skin needling of the face is used in two main situations. The first as an advanced skin treatment to maintain a youthful skin appearance or as a general rejuvenation treatment. The second is as a skin therapy for skin texture or scarring concerns. Skin concerns that have been treated with microneedling include acne scars, fine lines and wrinkles, enlarged pores, skin roughness, pigmentation and hair loss.

Face Needling for Prevention or Rejuvenation

Microneedling pens such as dermapen are most commonly used to maintain a youthful skin appearance and glow. These treatments employ hyaluronic acid, P.R.P. or mesotherapy to enhance the rejuvenating action. Made accessible by Kim Kardashian, who underwent “vampire facial” using P.R.P. and microneedling.

Skin Needling as a Therapy

Skin needling is an effective treatment for many skin concerns. The action of microneedling was first noticed when burn scars where being re-pigmented with tattoo guns. Not only where the white scars coloured to a more natural skin tone, but changes occurred to the quality of scar tissue. Since this time, microneedling has been used to improve the appearance of skin textural concerns.

Different Modalities for Skin Needling

Skin Needling with the Dermaroller

Dermarollers were the first skin needling devices after the tattoo gun. Dr Fernandes, a South African plastic surgeon (who was also responsible for skincare brand Environ) was the first to develop dermaroller technology. The first skin needling widely used.  However, the rolling technique did cause excessive trauma to the skin when needling to deeper levels. They remain a home treatment with the external application only.

 

Microneedling Pen such as Dermapen

Dermaroller treatment was surpassed with the introduction of Microneedling pens. Dermapen was the first; however, there is now a wide range of these. These have gained a central place for face needling and rejuvenation. These can be combined with P.R.P. or mesotherapy such as NCTF. The skin needling creates a channel to allow the introduction of growth factors, hyaluronic acid or mesotherapy components.  Care must be exercised with these. They are not suitable for deep microneedling, which again can risk the formation of tiny scars. Instead, they are best used with up to 1mm depth for prevention and skin texture improvement.

 

Rf Microneedling for Advanced Skin Treatments

The last evolution for skin needling is rf Microneedling. The first introduced onto the market was ePrime; however, this was quickly surpassed with more predictable devices, including Infini and Secret R.F. The introduction of rf microneedling was a game-changer as it combined two beneficial skin treatments. I.e. radiofrequency and microneedling. Rf microneedling allows a controlled depth of penetration as well as a controlled energy release deep in the skin. It is now considered the single most useful treatment for most types of acne scarring. It can be combined with CO2 laser or other treatment modalities.

What does Microneedling Treat?

Microneedling is most commonly used for acne scars; however, it can also be used in the treatment of:
• Skin texture concerns, fine lines wrinkles, enlarged pores, sun-damaged skin
• Stretch marks
• Chickenpox scars

Skin Needling for Acne Scars

Skin needling has established place and effectiveness for treatment of some acne scars. Advanced rf microneedling gives the most impact, though the use of a microneedling pen and home use of a dermaroller are still employed. If your clinic has rf microneedling, this will provide the most significant therapeutic benefit. Remember with acne scar treatment; several different treatment approaches are often required. An acne scar treatment plan can include more than one treatment with T.C.A. cross, subcision, dermal filler and laser in addition to rf microneedling.

Skin Needling for Stretch Marks

Once again rf microneedling is considered superior to microneedling pens or dermal rollers in treatment of stretch marks.  Rf microneedling can be used for young or old stretch marks and should be combined with laser treatment. For young stretch marks vascular lasers, and for old stretch marks, CO2 laser treatment improves the outcome of needling.  

 

Skin Needling for wrinkles and texture concerns.

Skin needling is useful for a range of skin texture concerns.  Once again, rf microneedling gives superior results, though dermapen also provides significant results for superficial texture concerns. Dermapen needling is combined with either P.R.P. or NCTF Mesotherapy is a more advanced skin treatment.

Fine lines and wrinkles, crepiness, enlarged pores, skin roughness and even dryness, are improved with a series of microneedling treatments. Caution must be exercised as excessive use of deep (3mm) needling with microneedling pens has resulted in scarring.

 

Microneedling for Hair Loss

The home dermaroller has been widely used for hair loss. However, microneedling only increases vellus hair and not terminal hair growth. So, though dermaroller can give an increase in fine hair, this does not lead to sustained hair regrowth. Treatment of hair loss is more complex, and though no harm will be done, equally, there is no benefit.

Microneedling for Pigmentation

Do not use microneedling to treat melasma. Overzealous use of skin needling fact this can result in worsening of melasma. Very superficial microneedling with pen technology can help with superficial pigmentation because it acts as a peel. However, be very cautious of allowing anyone recommending microneedling as a treatment for pigmentation.  It is not the most effective treatment for this, and in the wrong hands can cause problems.

 

How does Microneedling rejuvenate your skin?

Microneedling is a collagen induction therapy, i.e. it has a therapeutic effect by inducing collagen remodelling. Collagen induction therapies include rf microneedling, fractional CO2 laser,  Fraxel laser,  Dermapen and dermaroller skin needling.

What Is Collagen Induction Therapy?

All collagen induction therapies (C.I.T.) work by laying down new collagen to improve the appearance of the skin. The first step is to break down the scar collagen, releasing growth factors which stimulate fibroblasts to lay down new healthy collagen. The process is a controlled injury to the skin.  The C.I.T.’s used for acne scars include microneedling, radiofrequency and laser. So, whether it is a fractional CO2 laser, radiofrequency microneedling they all act by first breaking down the scar collagen, so your body’s natural repair can create healthier new collagen. Microneedling and laser for acne scars vary in the degree of effectiveness.

Most Effective Collagen Induction Therapy for Acne Scars

  1. Radiofrequency microneedling such as Infini and Secret rf: – This treatment works by inserting needles under the skin and then discharging radiofrequency in the dermis. It can surpass the effectiveness of lasers for some types of acne scars.
  2. The laser has long been the most effective treatment for deep and severe acne scars. Fractional CO2 laser remains a useful tool for some forms of acne scars. However, radiofrequency microneedling is rapidly replacing even high-power CO2 laser treatments in many instances.
  3. T.C.A. cross and subcision: Though not strictly C.I.T.’s they are the most effective treatments for some forms of acne scars.
  4. Sublative rf. E Matrix and another dermal stamping radiofrequency treatment. These treatments are valid for new acne scars. However, for established and deep acne scars, you will need a higher-powered treatment such as rf microneedling or lasers.
  5. Dermapen: This is a more advanced form of microneedling for acne scarring. It is more controlled and once again effective for some types of acne scars.
  6. Dermaroller: Skin needling with the dermaroller is an entry-level treatment. Dermarollers do work, but there are more advanced treatments that are not only more efficient but also work more rapidly. Home Dermarollers can now be purchased.

What Is Involved in Dermapen Skin Needling?

  • One hour before needling Topical Anesthetic is applied to numb the area
  • The small needle penetrates through the surface layer of skin
  • With proper numbing, you should feel tiny, except towards the end of treatment it can become bitey
  • immediately after treatment your face will be red and feel uncomfortable
  • The surface layers seal and heal quickly
  • A cosmetic makeup such as Oxygenetix can be applied directly after treatment if required
  • Over the next six weeks to 3 months, you will see the rejuvenation benefits of skin needling

What to Expect After Skin Needling?

  • There will be redness and swelling that last 1-2 days after treatment. Some people with more sensitive skin or with a tendency to swelling this can last longer.
    • Bruising is rare but occasionally does occur and can take up to 7 days to subside.
    • 3-5 days after there can be flaking or peeling of the skin.

The surface of your skin seals and heals quickly, after about a week your skin is pretty much back to normal. Under the skin, the process of collagen remodelling starts. The collagen induction peaks at 12 weeks but can continue for up to 12 months.

How Much Improvement Can I Expect with Skin Needling?

  • The results of skin needling are progressive over several treatments.
  • Generally, four treatments are recommended in a package, spaced four to six weeks apart.
  • The number of treatments depends on your response to treatments.

The results are individual and can depend on factors such as your general skin health as well as your intake of fresh fruit and vegetables. It is also best to take Vitamin C & Zinc before treatment can improve results.

What can you do to improve the outcome?

  • Use sun protection after treatments.
  • Start on prescription Vitamin A cream at least two weeks before treatment.
  • Eat a healthy diet with fresh fruit & vegetables.

What Are the Side Effects of Dermapen Skin Needling?

Side effects of skin needling are rare but can include:

  • Infection. If you have a history of cold sores, we ask you to take precaution beforehand.
  • Bruising is rare, and this can be lessened by taking arnica before if you tend to bruising, or use of Hirudoid cream once the skin has healed. (after five days)
  • Prolonged/delayed swelling is very rare. Swelling can be treated with an antihistamine.
  • Depigmentation or hyperpigmentation is also very rare but can occur.
